We welcome new patients - especially your friends or relatives. If you are
a new patient or would like to refer a friend, please call our 937-222-EYES
line to schedule first time appointments.
If you need to reschedule your appointment, please give us at least a 24
hour notice. Dial the same number that you used to schedule your appointment.
Billing Questions
Please direct all of your billing questions to 937-223-1411 between the
hours of 9:00 a.m. - 12:00 noon & 2:00 p.m. - 4:00 p.m., Monday through
Friday.
937-222-3937 or 937-223-9117 is answered 24 hours with a doctor on call
to handle emergencies. If the emergency is after hours, our answering service
will promptly notify the doctor.
If you have a medical eye emergency during office hours, please tell the
receptionist or nurse, so she can assist you.
When you call in with medical questions, we have specially trained ophthalmic
technicians to answer your questions. If they are unable to answer your
questions, they will consult with the doctor and have answers for you as
soon as possible.
Prescriptions
To obtain a refill on a prescription, please have your pharmacist call 937-223-9117-
Monday-Friday, 8:30 a.m. - 4:30 p.m.
Your medical record is confidential and will not be released to anyone -
including another physician - without your written consent. To release your
medical record, please ask the receptionist for a medical record release
form.
Smoking is strictly prohibited within any of our facilities .

Surgery Patients
Our surgery counselor will explain all special instructions and needs at
the time of scheduling surgery.
Medicare regulations require all surgery patients to have a current physical
examination prior to surgery. You may use your own medical doctor, or if
necessary, we can recommend a conveniently located physician who accepts
Medicare assignment. Please ask for assistance when you schedule your surgical
appointment.
Our equipment and methods are on the leading edge of tomorrow's ophthalmic
technology. We perform complete exams including diagnostic tests, medical
and laser treatments, and eye surgery. In some cases, additional special
tests may be necessary to evaluate your entire visual system and to determine
the most specialized treatment plan for your individual care.
We work with you to make the best possible eye care affordable
At Ohio Eyecare Specialists, treatment is highly specialized and uses the
most sophisticated tests available today. Although cost is associated with
the sophistication of current technology, our primary concern is to provide
our patients with the best possible ophthalmic care at a cost that will
not e a burden. We recognize that our patient's financial situations vary
as much as their health care needs. We will make every effort to work with
each patient on a case-by-case basis regarding cost. If at any time you
have questions regarding costs - especially prior to special testing - our
financial representative will gladly review potential costs and payment
policies.
Payment in full or proof of insurance is expected at the time services are
received. We accept personal checks, cash, Master Card, VISA, Discover,
& American Express.
We accept Medicare, Medicaid, and most other major medical insurance. If
you are unsure whether we are a provider for your insurance, please consult
your provider book, or call your benefits coordinator. Some insurance agencies
require a referral from your primary care physician. This is the sole responsibility
of the patient, failure to obtain the referral may result in nonpayment
of your services, by your insurance carrier. If you have not obtained your
referral at the time of your appointment, we will have to reschedule at
a later date.
Regardless of insurance coverage, however, the patient or legal guardian
is ultimately responsible for the financial liability of services rendered.
